Instructor Evaluations

 

Many UCSC departments use the UCSC Instructor Evaluation Form to elicit student ratings of teaching:

  • Side one of the form is machine-readable and includes space for department-specific issues
  • side two is reserved for students' written responses to open-ended questions.
  • CTL scans the machine-readable items and prepares a statistical evaluation report, which is returned as a PDF to the department (custom reports are generated by the departments from raw data files emailed in .txt format).

Departments interested in using the UCSC Instructor Evaluation Form are invited to view the sample packet. Please note that the PDF version is only a sample. To order forms for your department, email CTL at ctl@ucsc.edu or call x9-2351.

You will need Adobe Acrobat Reader to view the form. You may already have the program on your computer. If not, you may download it for free.

 

CTL is NOT currently accepting new departments at this time.
